Research has demonstrated that people with mental and substance abuse disorders may die decades earlier than the average person — mostly from untreated and preventable chronic illnesses like hypertension, diabetes, obesity, and cardiovascular disease that are aggravated by poor health habits such as inadequate physical activity, poor nutrition, smoking, and substance abuse. Barriers to primary care, coupled with challenges in navigating complex healthcare systems, have been a major obstacle to care.

Psychiatric Health

Services available

Community Health Associates provides psychiatric services for adult and adolescent clients who need psychiatric evaluation and medication management.  Medication management is the use of psychotropic medications based off of an initial evaluation which consists of your past history and current/past medications, while listening to your treatment goals to help improve your mental health.  Our clients follow up on a routine basis to monitor and adjust these prescriptions as related to their daily lives. 

One of the things that makes Community Health Associates unique is that we also have expertise in treating people with dual diagnoses (mental health and substance abuse disorders).  Psychiatric Medication Management services are offered in conjunction with other outpatient services or as stand-alone medication management only service.

Medication Monitoring

Medication Monitoring  Due to the nature of medications, dosing issues, and problematic side-effects, we monitor participants regularly. Our staff is educated about medications and understand what participants are likely to encounter. Our medication monitoring program works to reduce the risk for participants of misuse, abuse, or overdose of their medications.

Case Management

The Health Care Coordinator is the point of contact in the collaborative process of case management.  Case management consists of assessment, treatment planning, coordination of care, evaluations, and advocacy of options and services to the participants and family's health needs.  

Counseling Services

Counseling A collaborative effort between counselor and participant, either individual, couples, family, or in a group setting. In which identified goals and potential solutions to problems which cause emotional distress, seek to reduce symptoms by improving communication and coping skills, strengthen self-esteem, and promote behavior change and optimal mental health.

Judicial support

Community Health Associates offers assistance for individuals re-entering the community from the justice system.

In an effort to assist our members to and from scheduled appointments for their services, we gladly provide transportation based in need.

Reminder calls are provided to create a smoother process for those in need of transportation services.

Coordination of Care/Referrals

Healthcare providers work with several different agencies to help provide the best care to all enrolled members. At times, a member may need to be referred to a specialty provider in order to meet their current needs.

Community Health Associates will coordinate care for our members by assisting with the referral process,  ensuring that communication is open throughout the process.

Community Health Associates is proud to be partnered up with Genoa, to provide prescription services to all of our members.

Genoa is dedicated to serving the needs of those in the behavioral health and addiction treatment communities, and other who have complex, chronic health conditions.
